| Which insurances do you accept? |

We accept most insurances. We are contracted (i.e. "in network") with Delta Dental and BlueCross BlueShield of MA. We do not accept Fallon, MassHealth, Harvard Pilgrim, or plans where you must see a participating provider.
As a courtesy to you, we file your insurance claims and request payment of your benefits to our office. We advise you to understand your coverage, since each dental plan is unique.
Please keep in mind that dental insurance isn't really insurance (a payment to cover a loss) at all. It is actually a benefit provided by some employers to help pay for routine dental care. The benefit amount is determined by how much the employer is willing to pay toward the cost of the premium. Most benefit plans are only designed to cover a portion of the total cost.

| Do I need to pay anything at the time of my visit? |

On the day of your appointment, you are responsible for your deductible and a co-payment that we estimate your insurance will not cover. Any remaining balance after your claim is received is your responsibility.

| Do you have payment plans? |
|
We offer two payment plans to those who qualify. Through CareCredit® and Citi® Health Card, Kozica Dental offers a range of no-interest and extended payment options. If you qualify, you'll receive a line of credit and a credit card to use for your health payment needs. For more information about the specific payment options we offer, please call our office.

I'm a new patient; why can't I just get a cleaning? |
|
There are several types of "cleanings." A comprehensive exam is necessary first to determine what is appropriate for each patient. After the doctor determines what treatment you need, we'll schedule your cleaning and any other necessary appointments.
